Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

Most insurance companies will respond within four to six weeks of claim submission. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can assist you to make arrangements for a monthly payment plan thru an external financial company if necessary. If a payment plan is to be implemented, this must be completed prior to the actual procedure.
